We are a physician-led company that supports aging adults in Northern California. We are a very unique, highly specialized team: All we do is support individuals with dementia and cognitive decline and their caregivers in a wrap-around way. Services are completely free and covered by Medicare. Depending on the stage of dementia/cognitive decline, the needs are different. We get to know patients, caregivers and their story to tailor how we support them.
Our virtual Care Assistants (CAs) are an essential part of our care model - you are our welcome squad for caregivers who need help! You will help patients and their caregivers enroll in our free services to connect them to benefits. Responsibilities include calling and texting with caregivers and patients, collecting information from them about their situation and their needs, and recording it in online forms for our care teams to review. You get to join multi-disciplinary team meetings to discuss patient needs and best practices. Will require patience, empathy, clear and professional communication skills-no previous healthcare experience is necessary. You will be provided with company and dementia training, an online phone number to make calls from, and scripts to support your conversations with caregivers and patients.
You’ll help patients and families enroll in free services by:
- Calling/texting caregivers to understand their needs
- Collecting information via online forms
- Providing compassionate guidance through the enrollment process
- Occasionally joining team meetings to discuss patient cases
No healthcare experience required – we provide:
- Dementia care training
- Call scripts and resources
- Dedicated phone number for work
- Supportive team environment
- Up to 30 hours of combined project work available if desired
Ideally candidate is:
- Bilingual English speaker; Fluent in Cantonese and/or Mandarin
- Must be available six days per week, for a total of 6-12+ hours.
- On three of those days, extended availability is required to respond to calls and messages in real time
- On the remaining three days, availability for a minimum of 30 minutes is required to monitor and respond to messages.
