This is a 7.5" x 3.5" official Organ Donor Form with a detachable Mickey Mantle card that will tell whoever finds your dying body, that you want to donate your organs. The actual card is detachable from the larger card and measures 2.6" tall x 3.5" wide and is meant to carry in your wallet or purse.
Mickey Mantle received a liver transplant in June of 1995 and the Mickey Mantle Foundation was his way of contributing and returning to the people who idolized him for so many years.
Card is in Mint Condition
