WISTA WISTAR 130mm f/5.6 Stereo or ID Photo Lens for 4x5 EXCELLENT
I'm assuming this was part of an ID camera rig as there is only one shutter speed and it has a flash synch socket on the face.
It has f/stops from 5.6-32. It is mounted in a Linhof Technika-style lens board. There is a center septum that is spring loaded that presses against the film to keep the images separate.
