Homeward Bound is seeking a Coordinator for individuals and families who are working their way out of homelessness. This pivotal role involves supporting clients as they transition from homelessness to permanent supportive housing. Working closely with a dedicated team of providers, you'll tackle challenges head-on and maintain regular contact with program participants. Importantly, we are in the process of constructing 26 units and of Permanent Supportive Housing (PSH) for single adults and families, slated for occupancy in early 2025, and this role will play a crucial part in assisting clients during their transition and stabilization in these units.
Key Responsibilities:
Deliver intensive permanent supportive housing services for up to 24 single adults and 6 families per month
Conduct meetings at least monthly with clients to address and overcome challenges related to maintaining permanent housing, documenting these interactions in regular progress notes.
Assist the Program Director with client identification and screening for housing eligibility, leasing, and client move-ins
Assist the Program Director with developing program policies and practices, unit setup and purchasing, and facility troubleshooting
Assist with submitting documentation to the Marin Housing Authority for the issuance of client vouchers
Assist with property management, track client rental payments, document tenancy challenges, write and issue lease violations as needed
Represent the agency in an organized and professional manner at all times
Participate in providing CalAim services to an assigned caseload of clients, write client care plans, and submit billing information for CalAim participants accurately and in a timely manner
Collaborate with other Homeward Bound staff and agencies, participate in case conferences, and ensure the necessary documentation and HMIS data collection.
Partner with external agencies and other Homeward Bound staff to submit monthly, quarterly, and annual grant reports.
Attend weekly Coordinated Entry meetings.
Conduct outreach services as needed
Address all barriers to obtaining and retaining housing, such as chronic homelessness, job and/or income instability, mental health concerns, substance use disorders, under-employment, lack of income, interpersonal skills, meals, tenant responsibilities, and other obstacles.
Skillfully problem-solve urgent situations, including interactions with EMT and Fire, police, psychiatric services, and other community agencies and staff.
Prevent and/or resolve conflicts using a strengths-based approach.
Offer supportive counseling, guidance, and referrals to community resources.
Engage regularly with individuals to ensure program participation and progress.
Assist program participants in housing retention, job readiness skills, job placement and retention services, education on living skills, navigating resources and referrals, eviction diversion, and addressing physical and mental health issues.
Communicate effectively, both in writing and verbally, with team members, supervisors, volunteers, and program participants.
Maintain a daily log and comprehensive case notes, documenting relevant client interactions.
Enter, update, and confirm information in the Homeless Management Information System (HMIS) and other databases as required.
Attend weekly program staff meetings, biweekly department meetings, monthly all-staff meetings, weekly supervision with the Program Director, and training sessions.
Report program, client, and facility needs to the appropriate individuals.
Ensure thorough documentation within 72 hours of meeting with a program participant.
Perform other duties as assigned.

Foster open and collaborative relationships with colleagues, working as a cohesive team with all Homeward Bound staff and partner agencies
Greet, communicate, and treat all program participants with respect, dignity, and a service-oriented attitude.
Uphold strict confidentiality regarding all program participants.
Maintain clear and consistent communication with your supervisor.

Minimum professional requirements include:
Bachelor’s Degree in social work, social welfare, psychology, or a related field, or at least 4 years of experience in a related case management or housing navigation field.
3+ years of experience working with under-resourced, low-income populations; experience working with individuals experiencing homelessness is desired.
Experience in residential programs and/or housing programs.
Preferred: Experience working with underserved populations and knowledge of local client services.
Working knowledge of Social Security income (SSI, SSDI, SDI).
Knowledge of life skills, money management, and basic math skills.
Strong organizational, communication, and documentation skills.
Data system entry experience with a high level of attention to detail; HMIS experience is preferred.
Proficiency in Microsoft Office Suites software.
Comfort working in a culturally and ethnically diverse work environment.
Awareness of and adherence to agency’s JEDI (Justice, Equity, Diversity, and Inclusion) principles.
Strong skills in harm reduction and working knowledge of substance dependence disorders and people living with persistent and/or serious mental health diagnoses.
Ability to read and interpret documents, write routine reports and correspondence, and speak effectively before small groups of clients or employees of the organization.
Physical Requirements include:
Ability to sit and use a computer mouse, keyboard, and monitor for moderate periods of time (1 – 3 hours per day).
Occasional stooping, crouching, kneeling, and/or crawling.
Ability to lift and/or move objects weighing up to 40 pounds.
Enthusiasm for working with individuals with previous experiences of homelessness.
