Our Agency is looking for a FT RN (Monday through Friday) as a Clinical Case Manager at our Santa Rosa agency.
This is a fully benefitted position - 40 hrs / wk in a non Medicare Agency. You would be responsible for case managing both home care aide care and skilled clients in Sonoma County only. You would be joining a clinical team supported by a RN NP who owns the Agency. We will train the right person who joins our team....
This is not a remote position. Full benefit package offered which includes mileage, health, dental, 3 weeks PTO and Company credit card for expenses.
