Looking for that special RN to join us as a Clinical Case Manager at our Santa Rosa agency Monday through Friday. This is a 40 hr / wk position to case manage our clients. We provide home care aide care and skilled services (non Medicare), as well as Pediatric home care in Sonoma County only.
We are nurse owned and family run agency that chose not to participate in Medicare. So, we have the best of both worlds - providing caregiver services to our elders in Sonoma County and the legal ability to also provide skilled services without the paperwork of Medicare! Our Agency is busy and needs another nurse to join our team. You would manage both home care aide type clients and some skilled clients.
