We are currently seeking an on-call licensed nurse to be back up if for any reason our staff needs additional support. This part-time position plays a vital role in ensuring our nursing shifts have available support after hours if needed.
Position Summary
The On-Call Nurse provides after-hours clinical support and coordination for home health patients. This position ensures timely response to client, caregiver, and physician inquiries during evenings, weekends, and holidays. The nurse serves as a resource for triage, urgent care coordination, and field staff guidance to maintain quality and continuity of care.
Key Responsibilities
Respond promptly to after-hours calls from clients, families, caregivers, and physicians.
Provide clinical triage, assess urgency, and determine appropriate interventions.
Coordinate visits for urgent care needs and communicate changes to the on-call administrator or DON.
Document all interactions, decisions, and follow-up actions accurately and promptly in the patient record.
Support field staff with clinical direction and guidance for patient care situations.
Notify DON or case manager of any significant changes in patient condition or incidents.
Participate in on-call rotation schedule and maintain availability during assigned hours.
Maintain confidentiality and compliance with all HIPAA and agency policies.
Qualifications
Current California RN or LVN license in good standing.
Minimum 1 year of clinical experience in home health, acute care, or related setting.
Strong clinical judgment, communication, and problem-solving skills.
Ability to work independently and manage multiple urgent situations calmly.
Reliable phone access and transportation if field visit is required.
Schedule and Compensation
On-call coverage for after-hours, weekends, and holidays as scheduled.
Compensation includes hourly standby pay plus per-call or visit rate.
