ESSENTIAL RESPONSIBILITIES: <br>
Patient and Family Care/Case Management<br>
•	Recognizes the patient and family as the “unit of care.”<br>
•	Completes Psychosocial Assessment in a timely manner and develops an individualized written plan of care.<br>
•	Provides social work services as outlined in the plan of care.<br>
•	Identifies and monitors potential psychosocial hot spots.<br>
•	Manages caseload of 25 for 1.0 FTE.<br>
•	Provides visit frequencies that are within established agency parameters and as outlined in the Plan of Care, appropriate to patient/family needs.<br>
•	Makes requests for overtime appropriately and in advance.<br>

•	Collaborates with IDG regarding changes in patient status and POC.<br>
•	Provides concise and timely reports to appropriate team members. <br>
•	Exercises professional management and coordinates care between and among involved parties in home, SNF and RCFE setting.<br>
•	Exercises good clinical judgment.<br>
•	Understands and properly utilizes changing network of community resources.<br>
•	Respects patient right to self-determination and acts as an advocate within the context of the values and ethical standards of HSCC.<br>
•	Assists Admission department with resourcing, assessment, information and registration process as requested.<br>

•	Completes information and registration visits for hospice or Transitions services, according to agency protocol.<br>

•	Collects and reports data on service preferences of key facility staff.<br>

<b>Duties and Responsibilities:</b>
<br>

•Timely review of prior authorization requests, both inpatient and outpatient;<br>

•Reviews prior authorization requests with (Associate) Medical Director as needed;<br>
•Prepares Notices of Action as needed meeting contractual deadlines and as determined by either DHCS or DMHC;<br>

<b>2.</b>Implements the Alliance Utilization Management Plan to assure appropriate use of medical services, coordinates a preauthorization review system for outpatient services and provides pre-authorization,  of acute care (inpatient) services, as required, by the use of established criteria, with duties including but not limited to: <br> 
•Performs preauthorization,  review for all hospital/SNF/Hospice and other LTC facilities utilizing the established criteria/protocols as approved by the plan;<br>
•Performs preauthorization review for all services, both in and outpatient , based on the Medi-Cal TAR list or the Plan TAR list utilizing criteria/protocols as established by the plan;<br>
•Reviews medical records to determine benefit coverage including appropriateness and level of care;<br>
•Updates and processes forms for member/provider files;<br>
•Assists Member Services, Claims and Provider Services departments with issues that require medical interpretation or definition;<br>
•Performs audits of medical records as needed; <br>
•Works closely with attending and contracting physicians, ancillary providers and county services  to coordinate member care.<br>
•Protect confidentiality of utilization review, quality management information and beneficiary identification;<br>
•Prepares written reports on the Alliance utilization information, including authorizations denials and use of covered services and other pertinent performance data, as directed.<br>
<b>3.</b>Participates in Quality Improvement Projects as needed.<br>
<b>4.</b>Assists the Prior Authorization Supervisor and department Directors in preparation for audits and other regulatory activities. <br>

In addition, this position is responsible for conducting oversight of the AB 1455 Claims Settlement Reporting and Provider Dispute Mechanism process, coordinating the Contract Implementation Committee (CIC) meetings, and assisting the Provider Services department with any contracting requests resulting from any State audit or license renewal.

<b>Duties and Responsibilities:</b>
<br>
<b>1.</b>Responsible for the oversight, coordination, implementation, processing, and maintenance of the provider contracting process with duties including but not limited to:<br>
•As directed, prepare all provider contracts and amendments for new and existing providers;<br>
•Coordinate, track and monitor all provider contract changes;<br>
•Update and maintain contract data in AIS appropriately;<br>
•Maintain system for tracking all provider contract information;<br>
•Follow up with providers to ensure the return of all new agreements/amendments and/or coordinate with PS Representatives as necessary;<br>
•Assist with contract implementation company-wide;<br>
•Research and respond to contract questions;<br>
•Coordinate contract/credentialing issues with Credentialing Specialist;<br>
•Maintain documentation for all contract types, amendment numbers and unique provider contract language;<br>
•Initiate, coordinate, and track state approval process of new contracts or amendments with Government Relations Director;<br>
•Maintain payment policy and payment matrix;<br>
•Oversee counter-signature process;<br>
•Create and maintain accurate contract files (hard and soft copies); <br>
•Audit the Provider Directory to ensure that current credentialing information is reflected; and<br>
•Write and maintain contracting policies and procedures based upon experience and/or evolving regulatory requirements.<br>

Implements the Alliance Utilization Review Plan to assure appropriate use of medical services, coordinates a preauthorization review system for outpatient services and provides pre-authorization, concurrent or retrospective review of acute care services, as required, by the use of established criteria.
<br>
<br>
<b>DUTIES AND RESPONSIBILITIES:</b>
<br>
•Performs preauthorization, concurrent and retrospective review for all hospital/SNF/Hospice and other LTC facilities utilizing the established criteria/protocols as approved by the plan;<br>
•Performs preauthorization review for all services, both in and outpatient , based on the Medi-Cal TAR list or the Plan TAR list utilizing criteria/protocols as established by the plan;<br>
•Interprets benefits based on the benefit language as outlined in Title 22, Provider Manual, and plan policies and procedures, to evaluate appropriate care for all lines of business;<br>
•Reviews medical records to determine benefit coverage including appropriateness and level of care;<br>
•Updates and processes forms for member/provider files;<br>
•Assists Member Services, Claims and Provider Services departments with issues that require medical interpretation or definition;<br>
•Performs audits of medical records as needed;<br>
•Works closely with attending and contracting physicians, ancillary providers, county services and institutional staff to facilitate discharge planning;<br>
•Protect confidentiality of utilization review, quality management information and beneficiary identification;<br>
•Prepares written reports on the Alliance utilization information, including admissions, discharges, authorization denials, use of covered services and other pertinent performance data, as directed.<br>
•Attends committee meetings as requested;<br>
•Performs other duties as assigned by the Utilization Management Manger or Medical Director.

<b>EDUCATION AND EXPERIENCE:</b>
<br>
•Current California Registered Nurse license;<br>
•Minimum three years experience in an acute care setting; <br>
•Minimum one year experience in Utilization Management, case management or equivalent.
<br>
<br>
<b>KNOWLEDGE AND ABILITIES:</b>
<br>
•Strong clinical nursing background;<br>
•Utilization Management experience;<br>
•Case management experience; <br>
•Ability to communicate effectively both orally and in writing;<br>
•Ability to use MS Word preferred;<br>
•Knowledge of Medi-Cal and related policy and Title 22 regulations;<br>
•Understanding of concepts pertaining to prepaid health care;<br>
•Ability to evaluate medical records and other health care data;
•Ability to exercise good judgment and tact in relating to contracting health care providers and beneficiaries;<br>
•Ability to establish and maintain effective and cooperative working relationships with Alliance staff and others contacted in the course of the work;<br>
•Ability to accurately complete tasks within established times; <br>
•Ability to effectively prioritize multiple tasks and deadlines;<br>
•Ability to maintain confidentiality;<br>
•Valid California Driver License, transportation and automobile liability insurance in limits acceptable to the Alliance.

Supervises prior authorization staff including Prior Authorization Nurses, CCS Liaisons, Transportation and Linguistics Coordinator and Utilization and Referral Coordinator. Ensures that prior authorizations staff are expediting requests in a timely fashion to meet contractual standards and ensures that all reviews are conducted using nationally recognized and evidence based standards such as the Milliman Care Guidelines; Performs duties as a Prior Authorizations Nurse; Participates in Quality Improvement Projects as needed; and Assists UM Director and department Director in preparation for audits and other regulatory activities.

<b>DUTIES AND RESPONSIBILITIES:</b>
<br>
<b>1.</b>Ensures that prior authorizations staff are expediting requests in a timely fashion to meet contractual standards and ensures that all reviews are conducted using nationally recognized and evidence based standards such as the Milliman Care Guidelines, with duties including but not limited to: <br>
•Ensuring that the Milliman Care Guidelines or other criteria adopted by the Alliance are used appropriately when applied to prior authorization requests;<br>
•Stays abreast of new developments in medical technology;<br>
•Maintains current knowledge of benefits and benefit changes in all Alliance lines of business;<br>
•Ensures that prior authorization staff remain aware of the varying benefits applied to different lines of Alliance business;<br>
•Advises the (Associate) Medical Director as to policy and regulatory issues pertaining to prior authorization mandates;<br>
•Educates providers, in conjunction with Provider Services staff, as to efficient and accurate methods of submitting prior authorization requests;<br>
•Ensures that staff maintains current knowledge of the Milliman Care Guidelines or other criteria adopted by the health plan;<br>
•Tracks turn around times (TAT) on prior authorization requests to ensure quality and remain within contractual mandates;<br>
•Evaluates Alliance staffing levels to ensure maximum efficiency;<br>
•Provides prior authorization coverage in the event of staffing shortfalls;<br>
•Investigates and follows up on complaints, grievances and quality issues related to the prior authorization process;<br>
•Ensures that members are notified of any denials in a timely and culturally competent manner in accordance with contractual mandates; <br>
•Ensures that members are notified of appeal rights and that staff distinguish between the Knox-Keene regulations and those mandated by the California Department of Health Services; <br>
•Investigates and follows up on complaints, grievances and quality issues relate to inpatient acute stays. <br> 
<b>2.</b>Performs duties as a Prior Authorizations Nurse, with responsibilities including but not limited to:<br>
•Timely review of prior authorization requests, both inpatient and outpatient;<br>
•Determines utilization decisions based upon nationally recognized guidelines adopted by the Alliance;<br>
•Maintains current knowledge of benefits for each line of business;<br>
•Reviews prior authorization requests with (Associate) Medical Director as needed;<br>
•Prepares Notices of Action as needed meeting contractual deadlines and as determined by either DHCS or DMHC;<br>
•Maintains current and thorough knowledge of Milliman Care Guidelines.<br>
<b>3.</b>Participates in Quality Improvement Projects as needed. <br>
<b>4.</b>Assists UM Director and department Director in preparation for audits and other regulatory activities.<br>
<b>5.</b>Performs other duties as assigned
<br>

<b>EDUCATION REQUIREMENTS:</b>

•Current California licensure as a Registered Nurse;<br>
•Minimum of three years three years utilization management experience, preferably in a managed care setting; and;<br>
•Experience pertaining to prior authorizations, health care quality improvement and/or clinical research preferred.
