The Team Leader (TL) is responsible for ensuring the delivery of high quality, cost effective end of life care to hospice pt/families primarily residing at home, through the coordination of daily clinical operations and the supervision and support of assigned work groups providing that care. TL ensures staffing coverage for direct contact with pts/families. TL Ensures accountability and compliance of team member’s to Hospice regulations and HSCC policies. The TL is responsible for the direction and management of the interdisciplinary team. The TL demonstrates: sensitivity and compassion for the issues involved in serving patients and families facing a terminal illness, death and bereavement; clinical expertise in pain and symptom control, management and leadership skills and fiscal responsibility in the planning and delivery of services.

ESSENTIAL RESPONSIBIILTIES:

PATIENT CARE MANAGEMENT
• Facilitates optimal interdisciplinary team process to ensure the delivery of high quality, cost effective care that maximizes patient care outcomes.
• Directs, coordinates and supports field staff members to promote optimal team functioning.
• Maintains expertise to serve as clinical and systems resource to field staff.
• Ensures utilization of resources, staffing to reflects needs of census across program functions.
• Develops systems & reports to enhance patient care and outcomes.
• Oversees patient care flow & response, priorities, assignments.
• Provides clinical direction and oversight of acuity ensuring optimal clinical outcomes and regulatory compliance. Names and monitors Hot acuity level situations, report F/U, tracks for trends.
• Manages/reports up appropriate clinical staff and system issues, concerns and suggestions to supervisor.
• Participates in the leadership, facilitation and documentation of department meetings as requested.
• Manages/reports up appropriate clinical issues to Medical Director. Maintains collaborative partnership.
• Participates in the leadership, facilitation and documentation of department meetings as requested.
• Ensures effective communication system with OnCall, Medical Director and Administrative OnCall for continuity and quality of care.
• Serves as liaison to Attending Physician as needed.
• Provides triage support to pt/families and staff when assigned triage staff unavailable.
• Directs IDG preparation and recertification review. Facilitates IDG meetings to promote patient-centered care planning.
• Provides accurate compliant documentation in medical record.
• Directs, manages and ensures documentation of Trigger Events related to symptom management & changes in level of care.
• Participates in QAPI monitors and activities.
• Responds in timely manner to Unusual Occurrence, Family Survey Feedback, Incident Reports and Complaints.
• Participates in Performance Improvement Projects and assigned workgroups to support culture of improvement and accountability.
• Assigns staff to ensure appropriate utilization in meeting departmental, interdepartmental & agency goals.
• Maintains positive and supportive working relationships with administrative and helpdesk team members.
• Works cooperatively with other nursing managers and department coordinators.

SUPERVISION
• Directs care provided by field staff members when appropriate.
• Ensures/develops staff accountability to identified outcomes.
• Ensures appropriate and compliant documentation per HSCC standards for assigned staff.
• Monitors patient care and provides individual and systematic feedback as needed.
• Maintains clinical expertise to lead teams to optimal clinical outcomes.
• Ensure timely performance evaluations with annual goals and professional development plan for each assigned employee.
• Develops outcomes, standards and program improvements for assigned staff.
• Monitors and verifies employee time worked. Identifies problems with timelogs, use of accrued time off, overtime and reports patterns of concern to supervisor.
• Reviews evaluations with supervisor and HR for input and direction.
• Advises supervisor of significant disagreements about performance.
• Develops corrective plans of action when needed.
• Participates in vacation planning/approval/coverage process.
• Reviews monthly reports of available sick and vacation time and discusses with staff, including advising of caps for sick and vacation.
• Coordinates any requests for leave of absence with Director of HR by requesting the LOA and ensuring completion and return of appropriate forms.. HR will facilitate approval process, advise and coach necessary steps and communication between supervisor and employee as needed.
• In case of an on the job injury, reports and completes supervisor’s section of the workers compensation claims and forward immediately to HR. HR will advise and coach – necessary steps and communication between supervisor and employee as needed.
• Develops understanding of budget process & maintains oversight and accountability for department spending.
• Engages actively in learning and growing in the field of management and leadership.
• Develops and utilizes understanding of statistical analysis in monitoring and establishing standards for productivity and other benchmarks. Reviews with staff routinely.
• Minimizes conflict and maximize support between and among other managers and teams.



REQUIREMENTS

EDUCATION/EXPERIENCE 3-5 years previous Supervisory/Management experience strongly preferred with Hospice management experience desired.

SKILLS REQUIRED: Critical thinking, good judgment, leadership, change friendly/adaptable, strong verbal and written communication skills, ability to bridge regulations with clinical practice, strong customer service, flexibility, and discernment in decision making. Ability to stand firm and remain committed when necessary and facilitate understanding when difficult or adverse decisions must be addressed or made.

CERTIFICATIONS, LICENSES, REGISTRATIONS:
Active State of Ca RN license, current drivers license required
Hospice and Palliative Certification preferred.
