RN CASE MANAGERS
We have immediate openings for RN Case Managers in Vacaville, Pleasant Hill and San Jose, CA!
Position Summary:
Reporting to the Clinical Team Manager, the RN Case Manager provides professional nursing services to an assigned group of patients and families in the home setting; responsible for on-going coordination and evaluation of care provided by multiple health care disciplines to ensure patients and families are provided information and access to all required services; promotes self-care, prevents episodes of illness and readmission among at-risk clients, when possible; assists patient and caregivers in determining health requirements and resources to remain in the home or for appropriate referral to placement or hospice; and plans and carries out an appropriate course of treatment to meet patient needs.
Minimum employment requirements:
(1) Current California RN license, (2) BSN/PHN preferred, (3) minimum 1 year acute care experience and current home care experience required, (4) BLS, (5) current California Drivers License and ability to drive to patients’ homes, (6) experience with OASIS for Medicare visits.
