Adult Health Care Center in Sunnyvale has openings for a MSW or a BSW with case management and geriatric experience.
JOB REQUIREMENTS:
• Evaluate/reevaluate each participant to assess his/her needs, and provide appropriate resources/community services
• Develop appropriate care plan and follow each case
• Maintain case records and documents in a timely manner
• Provide supportive counseling and advocacy for clients
• Work with participant’s families to ensure continuity of care
• Ability to work with a team
MINIMUM QUALIFICATIONS:
Master Degree or BS Degree in social work from an accredited school of social work is required. Medical MSW is preferred. Recent case management experience with geriatric experience is preferred.
Must have oral and strong written skills in order to prepare and present complete and concise written materials. Effective time management skills is a MUST.
