Primary Purpose: Under direct supervision by the Clinic Supervisor, this position provides administrative coordination for patients, physicians and practioners at the Institute for Health & Healing Clinic in San Francisco. Laguna Honda Hospital & Rehabilitation Center’s Palliative Care Spiritual Program is accepting applications for one (1) part-time (.6) position in class 2587 Health Worker III to function as the Palliative Spiritual Care Specialist. SHIFT: Mostly days. However, weekend and evening work will be required on a regular basis.
Laguna Honda Hospital & Rehabilitation Center is a 780 bed facility providing skilled nursing, intensive rehabilitation, limited acute and hospice care with a geriatric focus.
ESSENTIAL DUTIES:
Under direction, the Palliative Spiritual Care Specialist is a member of the interdisciplinary team, which interacts with hospital staff as well as members of the San Francisco faith community to connect individuals and resources to residents and their families in an effort to meet their spiritual needs and performs the following essential duties:
• Integrates spiritual services into clinical care and bereavement, facilitating appropriate spiritual counseling based on resident and family needs;
• Provides consultation with unit resident care teams;
• Refers residents to community spiritual advisers as-needed;
• Contacts family and loved ones immediately after the loved ones passing and integrates spiritual services into bereavement care as appropriate to family and caregiver needs;
• Organizes and conducts memorial services to meet the bereavement and spiritual needs of the affected staff, volunteers, family and loved ones of residents who have passed away;
• Establishes contacts and maintains relationships with the San Francisco faith community with the purpose of integrating individuals and groups into the provision of palliative care services and interventions at the hospital;
• Serves as a liaison to community spiritual advisers within the Bay Area, improving LHH/community integration;
• Acts as a liaison to hospital departments;
• Develops and coordinates the programming of the spiritual care for palliative care residents;
• Assists in developing a training curriculum for palliative spiritual care for staff and volunteers;
• Participates in departmental and hospital-wide performance improvement activities and contributes to the development of departmental policies and protocols;
• Provides in-service training to staff regarding the palliative spiritual care program at LHH;
• Maintains records of ongoing spiritual care services, i.e., updating Spiritual Assessment Forms and notating residents’ charts;
• As a member of the interdisciplinary team, attends hospice interdisciplinary resident rounds, ongoing team-building group, and administrative and monthly in-service training;
• Performs other duties as assigned/required.
MINIMUM QUALIFICATIONS:
Two (2) years of verifiable experience within the last seven (7) years, working in a health agency with a culturally diverse population providing health-related information, education, intervention and/or referral services; assisting clinical staff in the assessment process and providing input into the development of treatment plans; conducting intake/screening interviews; advising/counseling clients under the supervision of professional staff and participating in health promotion and health education activities.
Substitutions:
1. Possession of a baccalaureate degree from an accredited college or university in a job related health or social science field may substitute for up to one (1) year of qualifying experience.
2. Possession of a “Community Health Worker Certificate” earned from City College of San Francisco may substitute for up to six (6) months of qualifying experience.
DESIRED QUALIFICATION(S):
• One (1) year of direct resident/family care experience in a Hospice/Palliative care setting.
• Masters of Divinity degree or four (4) units of Clinical Pastoral Education or equivalent.

The Mental Health Specialist is responsible for coordinating client intake and admission as well as coordinating program and case management services with other services providers. The Mental Health Specialist also provides daily support to assigned physician.

SPECIFIC RESPONSIBLITIES:
The Mental Health Specialist shall:
* Support MD ACFE
1. Provide daily support to assigned Psychiatrist.
2. Complete initial intake/admission paperwork on all new clients. Develop a New Client Orientation Packet and ensure that at a minimum every client receives: Patient Rights Forms; New Clinic Packet; Clinic Policies/Procedures; After-Hour Telephone #s, etc.
3. Complete initial and annual ACFE’s (Annual Community Functioning Evaluation) on all clients within the program.
4. Coordinate with the Program Assistant and Billing Clerk to ensure that eligibility verification has been completed on each client seen each day.
5. Develop engagement strategies with all clients enrolled in the clinic (i.e. brokerage, linkage, milieu management).
6. Ensure that the waiting time for clients in the lobby is reasonable at all times in an effort to support each customer returning for services.
7. Manage the flow of clients for his/her assigned Psychiatrist for that day.
8. Document daily contacts with others involved in the client’s care (i.e. case managers, ACCESS, family members, residential care providers, guardians, hospitals discharge planners, etc.)
9. For all new clients, monitor client participation in treatment and design a client engagement system for immediate follow up in the event a client misses a scheduled appointment.
10. Assist in obtaining insurance cards; processing TARs; maintaining housing; and providing a list of resources for clients in need of these services. If working with children/adolescents, this also includes making contact with schools, courts, family services, etc. as needed.
11. On a daily basis, check each chart of clients seen that day to ensure that physicians, clinicians and clients have signed and completed all pertinent forms (medical consent forms; client treatment plan, etc.)
12. Work closely with Psychiatrists, Nurses, and/or Therapists on discharge planning for assigned client.
13. Complete required records documentation daily/weekly /annually and keep medical records and clinical documentation current at all times. All notes must be legible.
14. Attend staff meetings and communicate clinical concerns to team.
15. Participate in Continuous Quality Improvement Management team programs as requested by the Program Administrator. (i.e. distribute client satisfaction survey to each client at each visit to the clinic, monitor how many clients are being hospitalized each month, etc.)
16. Participate in special/emergency cases, case conferences, and client staffings as required.
17. Provide all clients with care, dignity and respect -- continuously striving to deliver the highest level of support and client-centered treatment.
18. Perform other duties as assigned by the Program Administrator.
