Progressive, Private Non-Profit agency seeks a Full Time Community Support Facilitator. This is a salaried, supervisory position, responsible for coordinating
support services for Supported Living Services clients on an assigned caseload (approx 3-4 clients living in Alameda County areas) We are looking for a super motivated person with great team building skills, solid supervisory and communication skills, to add to our existing team of supervisors.

Our ideal candidate will be highly motivated team player, successful supervisor with social services experience, who demonstrates follow through on assignments, has a clear understanding and commitment to the philosophy of inclusion, access and consumer-driven services. Has the ability to work cooperatively and effectively with consumers, their support systems, employers and other agencies. And has the proven ability to solve problems independently and as part of a team.

The ideal candidate will also have a valid California driver’s license, a driving record that meets agency standards, auto insurance and access to a vehicle, pass a DOJ back ground and reference check, under go TB testing and be willing to get CPR/1st Aid Certified within 30 days.

Salary is $2.500 for the 1st 2 months of training and then goes to $35k a year.

DUTIES AND RESPONSIBILITIES:

1. Assists/facilitates consumers to make fundamental life decisions while also supporting and facilitating the consumer in dealing with consequences of those life decisions.
2. Provides planning, assessment and ongoing monitoring to assure that SL Services are tailored to meet the consumer’s evolving needs and preferences for support.
3. Assists consumers to locate suitable housing, locate and choose suitable housemates and acquire household furnishings. Assists consumers in settling disputes with landlords.
4. Insures that training and support strategies are utilized to maximize consumer independence and/or participation in common daily living activities.
5. Assist consumer to participate in community life by facilitating consumer to explore options for activities & associations and to assist the consumer to access chosen activities.
6. Assists consumers in recruiting, screening, hiring, training and evaluating IHSS and direct service staff.
7. Assists consumers to build critical and durable relationships with other individuals and to build and maintain a circle of support consisting of a majority of members not paid to support the person.
8. Assist consumers to locate and schedule appropriate medical services. Responsible for assisting consumer to communicate changes in support to assist consumer to maintain health and well-being.
9. Assists consumers to deal with and respond appropriately to governmental agencies and personnel (e.g. Social Security, Housing Authority, Para transit, Department of Social Services (IHSS)).
10. Assists consumer to assert civil and statutory rights through self-advocacy.
11. Assists consumers to become aware of and able to effectively utilize the transportation, fire and emergency help available to the general public.
12. Ensures that all service and administrative duties are performed in a timely manner by direct service staff.
13. Participates in agency’s 24-hour emergency assistance system, which includes direct service in response to calls for assistance as well as assisting and facilitating the consumer’s efforts to acquire, use and
maintain devices needed to summon immediate assistance.
14. Participates in all required trainings and agency staff meetings.
15. Provides direct support services to person served, and/or services to support a consumer’s Individual Support Plan as needed.
