Salary Range: $3,530.09 - $4,290.85 per month

The TeenAge Program (TAP) is within the Public Health Division of Contra Costa Health Services. TAP currently provides comprehensive health education in school based clinics, high schools, juvenile detention facilities, and community-based organizations; facilitates youth access to health services through school-based health centers throughout the County.
Position Description:
Under Public Health Clinic Services, the Health Education Specialist (HES) works to increase youth access to medical services to local High Schools throughout Contra Costa County. The HES provides health education, outreach, and organizes day-to-day Mobile Clinic operations for their assigned High School(s). The position is based in Martinez and includes travel throughout Contra Costa County. This is a full-time, temporary position.
Specific Responsibilities:
· Serve as the primary contact between a mobile school-based health center and students and parents at High Schools in Contra Costa County. General duties include, but are not limited to: plan and conduct outreach with students and school staff regarding school based health center services; serve as a liaison between school sites and the mobile clinic; provide one-on-one health education and referrals; make appointments; work closely with other Clinic Services staff; enroll patients in appropriate insurance; follow up with patients; attend clinic meetings and trainings; and other clinic duties as assigned.
· Establish and maintain relationships with schools and community partners.
· Plan, conduct, and evaluate health education presentations for adolescents in Contra Costa schools, juvenile detention facilities and community agencies.
· Plan, conduct and evaluate on-going youth development programs.
· Participate in the development and assessment of health education materials and clinic services.
Position Requirements:
Candidates must possess a Baccalaureate Degree and one year of full-time experience in a health services agency or program performing health promotion, health prevention or community health education work or candidate must possess four years full-time as Community Health Worker II and 60 semester or 90 quarter units of college coursework with at least 18 quarter and 12 semester units in health related coursework, which may be substituted for the required Bachelors Degree. Candidates must possess a valid California’s driver license.
At least one year full-time experience in adolescent health education with high school youth is preferred, particularly in the area of comprehensive sexual health including pregnancy prevention and STD/HIV education. The ideal candidate will have a demonstrated ability to work with youth and adults, utilize youth development and youth organizing principles, be able to communicate effectively with people from diverse communities, work well independently and with a regular team and have experience working in a school based clinic or teen clinic setting, and write reports and keep detailed records. Fluency in Spanish is required/desired. The applicant must possess reliable transportation. Reimbursement is provided for travel between program sites.
